If a student requires a menu modification due to a disability, a meal substitution request form must be completed by a medical authority. Once the form is complete, it should be returned to the school secretary. The Child Nutrition Department will then be notified to provide reasonable meal accommodations. Please allow up to 10 business days for processing. The parent will be notified of the starting date for meal service.

Physician Request -Dietary Accomodations

For students with special dietary needs that do not have a disability, nutrition information can be provided to the parent upon request for the parent to determine which menu items the child will eat.
